Sorry for the delay, but I just found these questions...
m.5 - I play the Bb tremolo with RH (3&4), but the B tremolo with 2&3 and the upper notes with 4-4-3.
Next, after the D, 2(F#)-2(G)-134-2-234-2-4(grace note)123-2.
m.6 - 1st beat = RH upper staff, LH lower staff; 2nd beat = LH upper staff, RH lower staff; 3rd beat = RH upper staff, LH lower staff.
